177 A N A L E S R A N M R E V I S T A F U N D A D A E N 1 8 7 9 INFECCIÓN POR CORONAVIRUS EN LA GESTANTE Y EN EL RECIEN NACIDO José Antonio Clavero Núñez An RANM · Año 2020 · número 137 (02) · páginas 174 a 178 (26) han estudiado la patología de dicho órgano en 20 parturientas que eran positivas al coronavirus, y en 10 han observado alteraciones en los vasos fetales, incluyendo trombosis. Estos hallazgos podrían haber ocasionado, en los casos más manifiestos, una interferencia con la oxigenación fetal, y más aun al comenzar las contracciones del parto. Pero hemos visto que la pérdida de bienestar fetal no parece ser más frecuente en estas pacientes, por lo que estos hallazgos no pueden ser bien interpretados por el momento. Otro aspecto que plantean esta patología placentaria, es por que el COVID-19 no puede atravesar la finísima membrana sincitio-endote- lial que, llegado el término de la gestación, separa las sangres de la materna y del feto. Es probable que dicha membrana carezca de receptores para el coronavirus, y sea la explicación de que no halla transmisión vertical. Por último no se han encontrado coronavirus en la leche materna ( 13,27) aunque se ha descrito algún casos de infección tardía en un caso de fiebre alta (29). La lactancia materna no se aconseja para la Sociedad Española de Neonatología (30), aunque la madre podría extraerla hasta que su analítica sea negativa, iniciando entonces la alimentación directa de su hijo.
